- Step 1: Tell us how many participants are taking lessons.
- Step 2: Enter each participant’s basic information and parent/guardian details.
- Step 3: Submit the form, and you’ll then receive the official Liability Waiver by email for e-signature.
All waivers must be signed before the first lesson can begin.
