You are required to represent Dribbler in a professional manner at all times and work to the best of your ability. Do you understand and agree?

By checking this box, I acknowledge and agree that I am prohibited from soliciting Dribbler clients for private soccer lessons or engaging in any independent agreements outside of Dribbler. I will not approach, contact, or attempt to persuade any Dribbler clients to engage in services outside of the Dribbler platform or myself. I understand that violating this agreement may result in immediate contract termination and potential legal action.

By checking this box, I acknowledge and agree that at least one parent or adult guardian must be present at all times during every lesson I conduct. I understand and accept that I am not to be left alone with any minor under any circumstances. This policy is implemented to ensure the safety, security, and comfort of all participants, as well as to maintain professional standards.

I acknowledge that client offers are not guaranteed and depend on the availability of clients in my area. Once a client becomes available, I will be notified and will have the option to accept or decline the offer.

I acknowledge and agree that all payments from clients must be made directly to Dribbler. Dribbler submits payment to me directly after each soccer lesson and I understand that accepting payments directly from clients is prohibited under Dribbler's policy.

I understand I am an independent contractor and not an employee of Dribbler. I control how I deliver services, and I may accept or decline opportunities. Dribbler does not supervise or control my coaching methods.

By checking this box, I voluntarily authorize Dribbler to conduct a background screening as part of its onboarding and safety review process for independent coaches using the Dribbler platform.

- I understand that completion of a background check is required to be eligible to offer services through Dribbler, and that declining consent may limit or prevent access to the platform.

- I understand that this background screening may include, where permitted by law:

- National, state, and county criminal record searches (including felony and misdemeanor records and sex offender registries)

- Reference or experience verification

- Review of publicly available safety-related records or watchlists

I further acknowledge and agree that:

- Background screening results may be used by Dribbler solely to assess eligibility to participate on the platform and to support youth safety and trust.

- I certify that all information I have provided is accurate and complete to the best of my knowledge.

- Providing false, misleading, or incomplete information may result in denial or removal of access to the STXZ platform.

- Background screening outcomes may affect my ability to receive or accept opportunities through Dribbler.

By checking this box, I authorize Dribbler and its third-party screening partners to obtain and review information reasonably necessary to complete this process, in accordance with applicable laws.
